For repair / calibration requests we ask you to fill out this entire document and then Save and print it. Please send us a copy of this print together with your device(s) to our address mentioned on the Service Request document. Once we’ve received your shipment, we will send you a confirmation.

For shipments sent from outside the European union it is very important that the customs documents state clearly that these items are being returned to manufacturer for repair We will not accept any import duty charges caused by missing or errorneous customs documentation.
